Our services are tailored to the specific goals and requirements of each individual.

 

We work with each participant to get to know them, understand their background, learn about any challenges they are facing and put a plan in place to help them achieve their goals.

Community Nursing

Our Community Nursing Care service exists to provide support to participants with medical and healthcare needs so that they can remain independent within their own home. Our support ranges from personal hygiene management to assistance with administering medication to bowel management care and much more.

 

We tailor our services to suit the participants specific requirements, as we understand that managing an illness or disease can come with its own unforeseen challenges.

 

We want to help our participants learn to manage these challenges so that they can continue to live a healthy, positive life with the support they need.

Assistance with Travel and Transport

Having access to safe travel and transport allows participants to partake in activities outside of their homes and continue to work towards achieving their goals. We are here to provide support to get from A to B whether that be teaching participants the skills they need to catch public transport or organising a private ride such as a taxi.

 

Having access to these services means participants can get to where they need to be more freely and on their own terms, helping them to maintain their independence.

We are able to provide participants with the level of support they need  based on their requirements. We can accompany participants on their travels so that they feel safe and supported or we can drive them to where they need to get going. No matter what level of support is needed, we will align our service accordingly.

Daily Personal Activities

Our Daily Personal Activities support service provides participants with either supervision or assistance with managing their day-to-day life so that they can remain independent. Our team are flexible and tailor this service based on the participants preferences, goals and desired outcomes.

 

We can support participants with a range of different daily tasks from personal hygiene, including bathing, dressing and grooming, to meal preparation, to helping attend medical appointments and so on.

Household Tasks

Maintaining a clean, safe and healthy environment can sometimes be difficult for individuals living with a disability. We are here to provide care and support to participants who need it so that they can remain independent within their own home in a happy environment.

 

We are able to help with domestic chores that participants cannot do themselves such as:

​

  • Cleaning and maintenance

  • Laundry

  • Ironing

  • Dishwashing

  • Gardening

Community Participation

Participating in different activities within the community is a great way for participants to build their confidence and remain independent. It can be a great way to meet new people and make new friends, learn and new skill and try something different.

 

At Hopekare, we are here to make sure our participants feel supported when they attend an activity so that they can get the most out of it. We can help participants connect with activities of interest and accompany them to and from these activities to create a hassle-free, safe experience.

 

Examples of activities are:

  • Personal development courses

  • Going to the local library

  • Joining a social group

  • Going to the movies or theatre

Development of Daily Living and Life Skills

Our Development of Daily Living and Life Skills service focuses on teaching participants the skills they need to remain as autonomous as possible. The skills taught allow participants to deal with day to day life and manage challenges as they arise so they can build their confident, yet feel that they have the support they need.

 

We tailor this service to the participants specific goals and what they want to achieve. Whether that be learning how to catch public transport, meal preparation or getting themselves dressed.

 

No matter what the participants goals are, we work with them and their support network to implement a plan that leads to achieving their goals.

Supported Independent Living (SIL)

Hopekare provides support to participants living either independently or in a shared living environment. Living independently can come with its challenges and obstacles, and we are here to help participants navigate these and learn to manage them so they can remain confident and independent within their living circumstances.

There are a range of tasks that we can assist with to ensure participants remain happy within their environment. From household tasks to personal care needs, we have our participants covered no matter the level of support they require.

​

We work with our participants to set a plan in place that provides them with the support they need but also teaches them the skills required so that they build their capacity and can achieve their goals.
